Additional Instructions: A copy of the entry will be emailed to the Submitter's email address. Please provide two (2) copies of this entry form with your entry and proof of payment. The correct entry fee must accompany each entry. Payment must be made by Check or credit card. All entries must be submitted online.

Entries with proof of payment must be received at the NATAS Lone Star Chapter office no later than June 18, 2010 at 3 pm.
